Specific Situation Reported: Reporter states client is a widow whose husband died two years ago. She reports client has been going steadily downhill since then. Client no longer goes to church or the Senior Citizens Center. Reporter states client’s appearance has become dirty and soiled. She also reports client has quit going to the doctor and purchasing her medications.
