Referrals on Special Populations

3. Referral regarding structural concerns.Special Health Services

Send letter to SDOH, Special Health Svcs, telephone emergencies.
4. ANE referrals in which the AP is NOT a staff member.APSFollow procedure given in policy, protocols. Route letter giving findings to SDOH.
5. SN referrals in which the AV is in a NH.APSFollow routine procedure, route letter on findings to SDOH.
6. Referrals on patient-to-patient abuse.APS: Consult with LTC staff to determine level of supervision provided by the NH.If LTC accepts referral, follow procedures as in 2. If APS investigates, follow procedure as in 4.
Nursing home referrals: Call the SDOH and notify them of the referral.

Route memo of findings to DDS QA on Sheltered Workshops, group homes, which are licensed by DDS. Send results of referrals to OCA on Hissom class members. Route copy of APS-2 to Adult Day Care Program in Aging Services Division.
2. ANE referrals in which the AP is another resident or someone from another facility.APSFollow procedure as in 7. If inadequate supervision by staff is the issue, the facility is considered an AP as well as the person perpetrating the ANE.
3. SN referrals in which the AV in non-compliant with the care plan.APSFollow procedure as in 8.
4. Abuse RegistryAPSOCA (change in the law).
RCF, Group Home, Home Health, Geriatric Day Care, Assisted Living, and Shelter referrals

General poor patient care or poor environmental conditions but no specific patient is named. This includes complaints, such as the facility is understaffed, linens are dirty, patients are not properly fed or groomed, or air-conditioning or heating is inadequate.Oklahoma State Department of Health (OSDH), Protective Health ServicesThe Long-Term Care Investigations (LTCI) Section secures all pertinent information and telephones phones, faxes, or e-mails the information to the Oklahoma State Department of Health (OSDH), Protective Health Services.
Maltreatment referrals by a facility employeeLong-Term Care Investigations (LTCI)APS specialist refers allegations of maltreatment of nursing facility residents by facility staff to Long-Term Care Investigations. These referrals are also sent to Oklahoma State Department of Health (OSDH), Protective Health Services, Medical Facilities Service.
Maltreatment referrals in which the AP is not a facility employeeAPSAPS specialist enters all pertinent information in the APS Computer System Intake Section and submits the referral for assignment. APS staff also phones, emails, or faxes a summary of the referral to Oklahoma State Department of Health (OSDH), Protective Health Services, Medical Facilities Service.
Maltreatment referrals in which the AP is also a facility resident.Long-Term Care Investigations (LTCI)When the AP is also a facility resident, the AP is entered as an alleged victim (AV) in APS intake. The intake is assigned to Long-Term Care Investigations.
Violations of residents’ rightsOklahoma State OmbudsmanWhen there are violations of residents’ rights, a referral is sent to the Oklahoma State Ombudsman office.
Allegations of Maltreatment of a Resident of a Nursing Facility, Intermediate Care Facility for Individuals with Intellectual Disabilities, or Oklahoma Veterans Nursing Facility
