Interview alleged perpetrator regarding specific allegations.
Interview other staff as needed to serve as collaterals.
Observe and document conditions of the facility and what clients and staff were engaged in at the time of the visits.
Review transportation arrangements, supervision, work requirements, hours, etc.

Alternative means of discipline
Advising facility of civil rights violations
Suggestions for improved relations between facility and participants
Special consideration:
Results of the investigation may not be released without court order except to the facility director.
Reports of this type may be politically charged. Care, tact and diplomacy are needed. Insure information is kept confidential. Discuss report with no one other than supervisor.
Determine if any DHS staff are on the board of directors or advisory boards for the facility.
Contact District Supervisor, or APS Field Representative with unusual developments.
